I ask for payment for the session at the time of service, or for the total amount due at the end of each month. I will then give you a statement which you can submit to your insurance for reimbursement. Please talk to me if you have special circumstances and need special arrangements for payment or for your insurance submission.
If you need to contact me, I request that you do so by telephone at (408-358-4090) as I do not have email capability at the office. I generally return telephone calls during weekday office hours. If I am out of the office for evenings or weekends, I do check my messages periodically and will return your telephone call if necessary. If I am on vacation, I will have a colleague on call for me and if necessary, I will arrange for someone to see you in my absence.
If at any time you have an emergency, and cannot wait for my return call, please call the crisis hotline at (408-279-3312) or go to your local hospital emergency room.
