Policies
I ask for payment for the session at the time of service, or for the total amount due at the end of each month. I will then give you a statement which you can submit to your insurance for reimbursement. Please talk to me if you have special circumstances and need special arrangements for payment or for your insurance submission. Information about paying securely with Pay Pal is located here, or with the link at the bottom of the web page.
If you need to contact me, you may do so by telephoning me at (408)359-4090. I do have email capability at the office, but please be aware that email is not considered secure and medical confidentiality can be compromised. I generally return telephone calls during weekday office hours. If I am out of the office for evenings or weekends, I do check my messages periodically and will return your telephone call if necessary.
If I am on vacation, I will have a colleague on call for me and if necessary, I will arrange for someone to see you in my absence. If at any time you have an emergency, and cannot wait for my return call, go to your local hospital emergency room or call the Crisis Hotline at (408)279-3312. Ethical standards
I take the ethical standards of my profession seriously and have served as an instructor psychology graduate students and chaired the ethics committee for the California Division of AAMFT (see my resume).
You should know that information disclosed by you during the course of therapy is strictly confidential unless you sign a written consent form for disclosure of information. There are certain legal exceptions that are described in HIPAA (the Health Insurance Portability and Accountability Act of 1996). |
